Amendments
Please be aware that changes have been made to passport amendment procedures. As of October 25, 2005, we can no longer print amendments into passports locally; rather, all changes will require a replacement passport.
To make changes to your personal information in your passport, or to add pages, you will need to bring your current, valid passport to the Consular Section, along with certified documentation of your data change. The proper forms are available at the Embassy, or you can print and fill out the correct form by following the links below.
Making Changes to Personal Information
If:
• Your passport was issued less than one year ago, and you need to make changes to the personal information, you will need to use Form DS-5504. There is no replacement fee.
• Your passport was issued more than one year ago, and you need to make changes to the personal information, you will need to use Form DS-11 or Form DS-82, depending on the age of the applicant. The fee for applicants under age 16 is $85, or $75 for applicants over 16.
Please allow about two weeks’ processing time.
Adding Pages
To add pages, you will need to fill out Form DS-4085. This process can be completed for next-day pickup.
