Passport Visa Pages
Only one set of extra pages (24 pages are in each set) can be added to your passport at a time.
If you wish to add pages to your passport, please submit:
- Your valid, undamaged passport
- Passport form DS-4085: Application for Additional Visa Pages.
- Fee is $82.00.
Passport services are done by appointment only.

For better service please have your passport application complete with photographs 15 minutes before your appointment.
Please note that you may be asked to reschedule your appointment if you are more than 30 minutes late.
