Protect yourself from identity theft.  If your passport with a valid U.S. visa is stolen or lost, even if you have no immediate travel plans, please report the theft immediately to the Embassy by sending a copy of the police report and a letter containing your previous passport information by clicking here.

You must request a visa appointment for a new visa by obtaining a PIN and calling the visa appointment and information service at 705-3007.  You need bring a copy of the police report and an original Movimiento Migratorio with you to the visa interview.  You may obtain a Movimiento Migratorio from the Peruvian Immigration Service, DIGEMIN, 330-4066. Please note that once your passport with a U.S. nonimmigrant visa is reported lost or stolen to the Embassy, the visa will be electronically cancelled and you will not be able to use that visa to travel to the United States even if it is later found.
