Lost or Stolen Passports
Protect yourself from identity theft. If your passport with a valid U.S. visa is stolen or lost, even if you have no immediate travel plans, please report the theft immediately to the Embassy. You may do so by sending a copy of the police report and a signed letter containing your previous passport information (both as attachments), to the Non-Immigrant Visa Section email: LimaNIV@state.gov.
In order to apply for a new visa, you need to bring a copy of the police report and an original "Movimiento Migratorio" to the visa interview. You may obtain a "Movimiento Migratorio" from the Peruvian Immigration Service (DIGEMIN). Their contact telephone number is 417-6900. Please note that once your passport with a U.S. non-immigrant visa is reported lost or stolen to the Embassy, the visa will be electronically cancelled and you will not be able to use that visa to travel to the United States, even if it is later found.
