LOST OR STOLEN PASSPORTS

Lost or stolen passports must be reported to the Consular section at the time of application for replacement. You must fill out an application, Form DS-11, and a Form DS-64, Statement Regarding Lost or Stolen Passport. The loss or theft of your passport must also be reported to the nearest police station in the area where the loss or theft occurred, along with a description of any other items missing in the same incident.

You will need to bring two passport-sized photos (5 cm x 5 cm) and a fee of $100 (age 16 and older) or $85 (15 and younger) to replace your lost or stolen passport. Please click here for additional information about lost and stolen passports.
