Lost or Stolen Passports
 

Lost or stolen passports must be reported to the Consular section at the time of application for replacement. You do not need an appointment to apply for a replacement of a lost or stolen passport. You may fill out an application, Form DS-11, and a Form DS-64, Statement Regarding Lost or Stolen Passport on line and print it out to bring with you to the Embassy or you may take a form from the waiting room in the American Citizen Services Unit and fill it out before coming to the window. Make sure you fill out the right form. The loss or theft of your passport must be reported to the nearest police station in the area where the loss or theft occurred, along with a description of any other items missing in the same incident.

You will need to bring a:

  • Passport-sized photo (5 cm x 5 cm)
  • Copy of the police report
  • Fee of $135 (age 16 and older) or $105 (15 and younger) to replace your lost or stolen passport.

Replacing Peruvian Immigration Entry Stamps

Please note that new U.S. passports issued to U.S. Citizens in Lima will need new entry stamps from Peruvian immigration, the Oficina de Migraciones. Travelers can obtain a new entry stamp at the airport. While exiting through the airport immigration travelers will be required to show a copy of the police report, if stolen, or old passport, if renewed.

Travelers should be aware that Peruvian immigration authorities do not permit departure from Peru until all immigration matters, including fines and overstays, have been put in order.
