Please be aware that changes have been made to passport amendment procedures. As of October 25, 2005, we can no longer print amendments into passports locally; rather, all changes will require a replacement passport.

Passport services are done by appointment only. Please note that you may be asked to reschedule your appointment if you are more than 30 minutes late.

To make changes to your personal information in your passport, or to add pages, you will need to bring:

• Current, valid passport.
• Documentation of your data change.
• The proper forms depending on the change required. The forms are available at the Embassy, or you can print and fill out the correct form by following the links below.

Form for Making Changes to Personal Information

If:

• Your passport was issued less than one year ago, and you need to make changes to your personal information, you will need to use Form DS-5504.  There is no replacement fee.
• Your passport was issued more than one year ago, and you need to make changes to the personal information, you will need to use Form DS-11 or Form DS-82, depending on the age of the applicant. The fee for applicants under age 16 is US$105.00, or US$110.00 for applicants over 16.
