Making Changes to Personal Information
If:
- Your passport was issued less than one year ago, and you need to make changes to your personal information, you will need to use Form DS-5504. There is no replacement fee.
- Your passport was issued more than one year ago, and you need to make changes to the personal information, you will need to use Form DS-11 or Form DS-82, depending on the age of the applicant. The fee for applicants under age 16 is $85.00, or $75.00 for applicants over 16.
